DIRECT CONNECT Every 68 seconds, someone in America develops Alzheimer's. Unfortunately, there are many constraints on the health care system that make it difficult for people to get education and guidance when they're diagnosed. All too often, patients leave their doctor's office feeling scared and confused about what to do next. Even when doctors suggest that patients contact the Alzheimer's Association, patients and their families wait two years on average before making that call—two years when they could be making future plans and learning to live with their disease better. The goal of the "Direct Connect" campaign is to shorten the wait between diagnosis and help from two years to two weeks—giving families access to the information and support they need to take charge of their health and take steps to live their best lives. With Direct Connect, doctors refer patients, with their permission, to the Alzheimer's Association upon diagnosis. An Alzheimer's Association care consultant then calls the patient or their family member within two weeks to educate them about their diagnosis, help them create a plan and connect them with community resources and services. The Alzheimer's Association, through the Direct Connect campaign, plans to engage 100 new physicians, connect with 250 more individuals and families shortly after diagnosis and provide 450 additional care consultations. The campaign will fund this vital program from July 1, 2012, through June 30, 2013.
